Forms that make up the Product Listing Packet submitted by the vendor are:
- One-page introductory letter.
- Listing Submission Form
- Copy of Federal Label Approval
- Label and packaging approval / FMB reclassification packet
- Vendor Number Application/Update Form
- Vendor Address Information Sheet Form

Any additional information, pictures, or dummy bottles the company cares to submit.
